Anal Disorders: Piles, Fissures and Fistula
Piles or Bavasir - The inflammation in the veins of the anal tract is known as piles. They can be found inside or outside the anus. The size of piles can vary from person to person.
Fissure - A painful tear or cut around the anus is called fissures. Fissure can affect an individual of any age group but usually observed after the age of 50.
Fistula or Bhagandar - Fistula is a type of opening in the anal area filled with pus. Fistula is common among those people who earlier had anal pus or abscess that has not been healed in a proper manner.
About Piles
Anyone can get piles, but they are seen more in the old people. Haemorrhoids is another word used for Piles. Haemorrhoids is the collection of swollen and inflamed tissue around the anal and rectum. In most of the cases, piles don't cause any pain and symptoms and people don't even realise the presence of them. Haemorrhoids are painless, until their blood supply is interrupted.
Piles can be internal or external. The internal piles are generally located between 2 and 4 cm above the opening of the anus whereas the external piles occur on the outside edge of the anus. External piles become very itchy and painful if they have a blood clot in them.
Piles are categorised into four grades:
Grade I
Grade I piles are small inflammations inside the lining of the anus that are usually not visible. They may bleed but don’t come outside of anus.
Grade II
Grade II piles are bigger than Grade I piles. They remain inside the anus. They can come out of anus during a bowel movement but they go back inside on their own afterwards.
Grade III
Grade III piles are called as prolapsed haemorrhoids. They appear outside the anus. The affected person could feel them hanging from the rectum. They can be re-inserted physically by the person itself.
Grade IV
Grade IV piles are large and remain outside the anus. They cannot be pushed back inside and require proper care and treatment.
Causes
Piles are caused by increased pressure in the blood vessels in and around the anus. This pressure can cause the blood vessels in the anal tract to become swollen and inflamed. It can occur due to the following reasons:

A fissure is a split or tear that occurs around the anus and causes pain & bleeding during the bowel movements. They are common in infants and school aged children.
Fissures are of two types: acute and chronic. Acute fissures are easy to treat and also heal with time. Chronic fissures are more difficult to treat and can reoccur.

About Fistula
In fistula, small glands in the anus gets clogged, and become infected which causes pus or abscess. In this disease, a tunnel is formed, whose internal opening is in the anal canal and the external opening is in the skin which is close to anus.
