What are Thyroid Disorders?

What is a Thyroid Gland? 

• The thyroid gland is an endocrine gland in neck. It is a butterfly-shaped gland, which produces two hormones named- thyroxine (T4) and triiodothyronine (T3). These hormones are secreted into the blood stream by the thyroid gland. During infancy and childhood, adequate thyroid hormone production is essential for brain development.
• These two hormones are necessary for all the cells in body to work normally. They influence metabolism, growth and maturation of body tissues and body temperature. These hormones also regulates the speed of the body cells.
• When too much of the thyroid hormones are produced, body cells starts to work faster than normal. This condition results in a disorder called hyperthyroidism.
• Conversely if too little of the thyroid hormones are secreted, the cells and organs of body slow down and cause hypothyroidism.

1. Hypothyroidism or Underactive Thyroid

Hypothyroidism when thyroid gland starts producing an insufficient quantity of thyroid hormone. It usually develops from problems within hypothalamus, thyroid gland and pituitary gland. 

Signs and Symptoms of Hypothyroidism

• Fatigue
• Weakness
• Intolerance to cold
• Muscle aches and cramps
• Constipation
• Weight gain or difficulty in losing weight
• Dry, rough skin
• Eye and face swelling
• Depressed mood
• Excessive or prolonged menstrual bleeding
• Joint aches and pains
• High cholesterol levels
• Memory problems
• Thinning hair
• Slowed heart rate 

Causes of Hypothyroidism

• Thyroiditis: Acute Thyroiditis, Postpartum Thyroiditis or Hashimoto’s Thyroiditis causes Hypothyroidism. Among all of them, Hashimoto's thyroiditis is the most common cause of hypothyroidism in an adult. It occurs because of autoimmune destruction of the thyroid. This results in a decreased production of thyroid hormone.
• Anti-thyroid Antibodies: These are present in people who have type 1 diabetes, lupus, rheumatoid arthritis, chronic hepatitis, or Sjogren's syndrome. These antibodies cause reduced secretion of thyroid hormones because of thyroid destruction.
• Medications: Medicines such as lithium (Eskalith, Lithobid) causes drug induced hypothyroidism.

 2. Hyperthyroidism or Overactive Thyroid

Hyperthyroidism means production of too much of thyroxine hormone.

Symptoms and Signs of Hyperthyroidism

• Nervousness
• Irritability
• Poor concentration
• Reduced menstrual blood flow in women
• Racing heartbeat
• Heat intolerance
• Changes in bowel habits
• Enlargement of the thyroid gland
• Skin thinning
• Brittle hair
• Increase in appetite
• Sweating

 3. Goitre

It is a swelling in the front of throat. It is caused by enlargement of thyroid gland.

 Symptoms of Goitre

Goitres are generally symptom less. Some symptoms that might occur are:

• Swelling
• Feeling a constant pressure in the front of the neck that is worse when the head is tilted down.
• Coughing
• Hoarseness of the voice
• Difficult Swallowing
• Trouble breathing

 Causes of Goitre

• A lack of dietary iodine
• Age (Goitres are more common after age 40)
• Medical history
• Pregnancy and menopause
• Radiation exposure

Causes of Hyperthyroidism

• Graves' disease
• Toxic multinodular goitre
• Thyroid nodules that overexpress thyroid hormone
• Excessive iodine consumption
• Subacute thyroiditis
• Pituitary adenoma
• Drug induced hyperthyroidism (heart medication amiodarone (Cordarone)

4. Thyroid Nodules

Thyroid Nodules are the lumps or abnormal masses in the thyroid gland. They can be single or multiple and may vary in size. Nodules of the thyroid gland are benign (non-cancerous) in over 90% of cases. 

Symptoms of Thyroid Nodules

Thyroid nodules do not cause any noticeable signs or symptoms. Sometimes, nodules become large and they can be felt, be seen (often as a swelling), and press on windpipe (cause shortness of breath and trouble in swallowing)

In some cases, thyroid nodules secrete extra thyroxine hormone. This thyroxine can cause symptoms of hyperthyroidism like-

• Weight loss
• Increased perspiration
• Tremor
• Nervousness
• Irregular heartbeat 

Causes of Thyroid Nodules

Nodules are caused by Iodine deficiency, overgrowth of normal thyroid tissue, cysts, tumors or cancers.

5. Thyroid cancer

• Thyroid cancer occurs in the cells of the thyroid gland. Thyroid cancer is more commonly seen in adult women as compared to men and youth.
• Thyroid cancer occurs when cells in thyroid gland suffer from genetic changes (mutation). The mutations allow the cells to grow and multiply rapidly. The accumulating thyroid cells form a tumour. These abnormal cells can invade nearby tissue and spread throughout human body.
• There are several kinds of thyroid cancer. Category of cancer depends upon the specific cell type within the thyroid gland, which has become cancerous. Papillary thyroid carcinoma is the most common type of thyroid cancer. 

Symptoms of Thyroid cancer

• A lump, that can be felt through the skin on neck
• Changes in voice
• Difficulty swallowing
• Pain in neck and throat
• Swollen lymph nodes in neck 

Causes of Thyroid cancer

• Exposure to high levels of radiation
• Inherited genetic syndromes
